    it is a black control with two white buttons that has a
    long one piece cord
    that goes down through a hole in the chair and plugs into the motor with a round 90 degree, or "L" shaped plug, with five (5) pins.  There are MANY versions of this control out there, some with black buttons, some with pictures on the buttons instead of arrows, some shaped differently, but if you have a two button hand control with a long one piece cord, and it has a 5 pin connector on the end, then this one will be the correct one.  In fact, it works on ANY CHAIR that has a five pin connector (Catnapper used a control like this for awhile that had the one piece cord but had a straight connector on the end; this one replaces that; it looks a little wierd where it plugs in, but it is underneath the chair and no one will see it, and it works fine).
